How to Become a Patient

The services of the AIM Health Institute are provided on a first come, first served basis, except for the following patients who are given priority:

  • Patients with a terminal illness carrying a prognosis of life expectancy of less than 6 months, as determined by their treating medical provider.
  • Patients with severe symptoms or rapidly progressing medical illness. Their condition must be documented by a medical provider, and the provider must contact AIM Health to request that a patient be placed at the top of the waiting list.

Priority will be given to residents of Washington, D.C. and the metropolitan area.

To receive free or sliding scale (discounted) care, patients need to demonstrate both:

  • Financial need
  • Specific medical need
Financial need

All patients will be financially screened to determine eligibility prior to receiving charity care or a discount. Once approved, patients may qualify for charity care or a discount for up to one year, after which they will need to re-apply to continue receiving charity care. Discounts may be applied retroactively for up to six months if there are unavoidable delays in determining eligibility. In addition, interest-free payment plans will be offered to patients eligible for a discount and will be negotiable between patients and AIM Health.

Patients whose income is at or below 200% federal poverty income guidelines (FPIG) will receive 100% write off of the charges. For patients whose income is between 200% and 300% of the federal poverty income guidelines, a discount of 75% will be offered. Additionally, a discount will be available on a case-by-case basis, as determined by the AIM Patient Eligibility Committee, for patients whose income is below 400% FPIG and whose eligible medical bills exceed 30% of the patient’s annual family income.

Specific medical need

To receive free or discounted care, patients must provide documentation from their medical provider, with ICD-9/10 code, for at least one of the following conditions:

a. Any terminal illness with life expectancy less than one year, as determined by a medical provider.
b. Cancer, active or in remission for less than 5 years.
c. Any type of neurodegenerative condition: Multiple Sclerosis, Parkinson’s Disease, Alzheimer’s Disease, Amyotrophic Lateral Sclerosis, or dementia.
d. Other conditions deemed to be acceptable by a clinician affiliated with AIM Health Institute. For other conditions, a patient may need to be seen for an initial visit to evaluate specific medical need, and will be expected to pay out-of-pocket or through insurance reimbursement for this initial visit.

2. Income verification documentation (one of the following): most recent tax return, form W-9, paycheck stubs for 3 months, or a proof of Medicaid eligibility.
3. Documentation of specific medical need from your medical provider. If none, please specify that you are scheduling an initial evaluation visit (a charge might apply).
